Summary

NSAIDs such as nabumetone can blunt the antihypertensive effect of valsartan and hydrochlorothiazide. Together with a diuretic and an angiotensin receptor blocker, an NSAID raises the risk of acute kidney injury, particularly in elderly or volume-depleted patients, and may cause hyperkalemia.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Nabumetone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Nabumetone belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAIDs inhibit renal prostaglandin synthesis, causing sodium and water retention and reduced glomerular perfusion, which counteracts the antihypertensive agents and impairs renal function.

What to do

Use the lowest effective NSAID dose for the shortest time, ensure hydration, and monitor blood pressure, serum creatinine and potassium. Consider paracetamol or another alternative.
