Summary
Ribociclib is a moderate CYP3A4 inhibitor and may increase amlodipine concentrations. This can cause more hypotension, dizziness and peripheral edema. Ribociclib itself also prolongs the QT interval, which is exacerbated by thiazide-induced hypokalemia.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Ribociclib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Ribociclib belongs to Oncology.
Why it happens
Inhibition of CYP3A4-mediated amlodipine metabolism by ribociclib, with additive QT risk if electrolytes are depleted by hydrochlorothiazide.
What to do
Monitor blood pressure and edema and reduce the amlodipine dose if hypotension occurs. Check potassium and magnesium and monitor the ECG as recommended for ribociclib.
What to ask your pharmacist
