Summary
Combining a loop diuretic with a thiazide produces strong sequential nephron blockade and increases the risk of volume depletion, hypotension, hypokalemia, hyponatremia and renal impairment. Valsartan can aggravate renal dysfunction in a volume-depleted patient.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Torsemide belongs to Cardiovascular.
Why it happens
Additive diuresis and electrolyte loss from loop and thiazide diuretics, with ARB-related reduction in glomerular filtration under volume depletion.
What to do
Monitor electrolytes, renal function, weight and blood pressure after starting. Reduce diuretic doses if volume depletion or electrolyte disturbances develop.
What to ask your pharmacist
