What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Diclofenac can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Diclofenac belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, impairing ARB and diuretic effects and compromising glomerular filtration.

What to do

Avoid regular NSAID use where possible, using paracetamol or the lowest NSAID dose for the shortest time. Monitor blood pressure, serum creatinine and potassium, and ensure adequate hydration.
