Summary
Indomethacin can reduce the antihypertensive effects of olmesartan and hydrochlorothiazide. Together with an ARB and diuretic it increases the risk of acute kidney injury and hyperkalemia, especially in elderly or volume-depleted patients.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Indomethacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Indomethacin belongs to Musculoskeletal.
Why it happens
Inhibition of renal prostaglandins by indomethacin causes sodium retention and reduced glomerular perfusion, opposing the ARB and diuretic and impairing renal function.
What to do
Monitor blood pressure, serum creatinine and potassium, and ensure adequate hydration. Prefer a safer analgesic where possible, or limit indomethacin to the shortest course.
