Oxaprozin can blunt the blood pressure lowering effect of the olmesartan and hydrochlorothiazide components. In patients who are elderly, volume depleted or have existing renal impairment, the combination can also cause acute deterioration of renal function and hyperkalemia.

What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Oxaprozin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Oxaprozin belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandin synthesis reduces renal blood flow, causes sodium and water retention, and removes vasodilatory support that ARBs and diuretics rely on.

What to do

Monitor blood pressure, serum creatinine and potassium when starting or stopping oxaprozin, and maintain adequate hydration. Use the lowest effective NSAID dose for the shortest time, or choose an alternative analgesic such as paracetamol.
