Summary
Analgesic or antipyretic doses of aspirin can reduce renal prostaglandin synthesis, which may blunt the antihypertensive effect of perindopril and impair renal function in elderly or volume-depleted patients. The sodium bicarbonate content adds a sodium load. Low-dose cardioprotective aspirin has little effect.
What this interaction means in plain words
Amlodipine-perindopril and Aspirin / citric acid / sodium bicarbonate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Prostaglandin inhibition reduces renal perfusion and antagonizes ACE inhibitor vasodilation, with added sodium load from bicarbonate.
What to do
Monitor blood pressure and renal function when higher aspirin doses are used regularly, and keep patients hydrated. Prefer paracetamol for routine analgesia where appropriate.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
