Summary

Heparin can suppress aldosterone production in the adrenal gland, and ACE inhibitors such as perindopril also reduce aldosterone. Together they can raise serum potassium, particularly in patients with renal impairment, diabetes or other risk factors for hyperkalemia.

What this interaction means in plain words

Amlodipine-perindopril and Heparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-perindopril belongs to Cardiovascular; Heparin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive suppression of aldosterone secretion by heparin and ACE inhibition leads to potassium retention.

What to do

Monitor serum potassium and renal function when heparin is given with perindopril, especially if therapy exceeds several days. Treat hyperkalemia if it develops and reassess the need for either drug.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
