Summary
Belzutifan is a CYP3A4 inducer and may lower apalutamide exposure. Apalutamide, as a strong CYP3A4 inducer, may in turn reduce belzutifan levels, which are cleared partly by UGT2B17 and CYP2C19 and CYP3A4.
What this interaction means in plain words
Apalutamide and Belzutifan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Apalutamide belongs to Endocrine; Belzutifan belongs to Miscellaneous.
Why it happens
Both agents induce CYP3A4, and belzutifan reduces exposure of CYP3A4 substrates such as apalutamide.
What to do
Monitor efficacy of both drugs, including PSA response for apalutamide and tumor response for belzutifan. Adjust doses based on clinical response.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
