Summary
Nifedipine is a CYP3A4 substrate and aprepitant, a moderate inhibitor, may raise its levels. Hypotension, flushing, peripheral edema and tachycardia may be more pronounced. Nifedipine has little effect as a CYP3A4 inhibitor on aprepitant.
What this interaction means in plain words
Aprepitant and Nifedipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Aprepitant belongs to Gastrointestinal Tract; Nifedipine belongs to Cardiovascular.
Why it happens
Aprepitant inhibits CYP3A4-mediated metabolism of nifedipine.
What to do
Monitor blood pressure and heart rate during and after aprepitant dosing and reduce the nifedipine dose if hypotension occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
