Summary

Baclofen and meperidine both depress the CNS, so combined use can cause marked drowsiness, weakness, hypotension and respiratory depression. The effect is more pronounced in older or renally impaired patients because baclofen accumulates.

What this interaction means in plain words

Baclofen and Meperidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Baclofen belongs to Musculoskeletal; Meperidine belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive CNS and respiratory depression via GABA-B agonism and opioid receptor activation.

What to do

Use the lowest effective doses and monitor sedation and respiratory rate, especially at initiation. Reduce the baclofen dose in renal impairment.
