Summary

Both agents prolong the QTc interval, and their combination may increase the chance of serious ventricular arrhythmias such as torsades de pointes. The risk is greater with hypokalemia, hypomagnesemia, bradycardia or other QT-prolonging drugs.

What this interaction means in plain words

Bedaquiline and Lapatinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Bedaquiline belongs to Anti Bacterial; Lapatinib belongs to Oncology.

Evidence level: Moderate

Why it happens

Additive delay of ventricular repolarization from effects on cardiac ion channels.

What to do

Perform ECG at baseline and regularly during co-therapy, and correct electrolytes before and during treatment. Stop one of the drugs if QTc exceeds 500 ms or syncope or palpitations occur.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
