Summary

Drospirenone has antimineralocorticoid activity and can raise serum potassium. ACE inhibitors also reduce aldosterone and retain potassium, so together they may cause hyperkalemia, especially with renal impairment or other potassium-raising drugs.

What this interaction means in plain words

Benazepril and Ethinyl estradiol-drospirenone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Benazepril belongs to Cardiovascular; Ethinyl estradiol-drospirenone belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive potassium retention through drospirenone aldosterone antagonism and ACE inhibitor suppression of aldosterone.

What to do

Check serum potassium during the first treatment cycle and periodically thereafter. Avoid in patients with renal impairment or on other potassium-sparing agents, and consider another contraceptive.
