Summary

NSAIDs can blunt the blood pressure lowering effect of ACE inhibitors. In patients who are volume depleted, elderly or have reduced kidney function, the combination can cause acute kidney injury and hyperkalemia.

What this interaction means in plain words

Benazepril and Mefenamic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Benazepril belongs to Cardiovascular; Mefenamic acid belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandin synthesis reduces vasodilation and sodium excretion, opposing ACE inhibitor effects and reducing glomerular perfusion.

What to do

Prefer a non-NSAID analgesic where possible. If combined, ensure hydration and monitor blood pressure, serum creatinine and potassium, especially at initiation.

What to ask your pharmacist
