Summary

Sucralfate contains aluminium, which can bind bictegravir in the gut and reduce its absorption. This may lower bictegravir exposure and risk virologic failure and resistance.

What this interaction means in plain words

Bictegravir and Sucralfate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Chelation of bictegravir by polyvalent cations (aluminium) in the gastrointestinal tract, reducing absorption.

What to do

Take bictegravir at least 2 hours before sucralfate under fasting conditions, and avoid taking them together or sucralfate shortly before bictegravir. Consider an alternative gastroprotective agent if separation is not feasible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
