Summary
Lonafarnib is a strong CYP3A inhibitor and may increase bortezomib exposure, worsening neuropathy and cytopenias. Lonafarnib is itself a sensitive CYP3A substrate, so inhibition by bortezomib is unlikely to be clinically relevant. Lonafarnib's QT effect is also unknown.
What this interaction means in plain words
Bortezomib and Lonafarnib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
