Summary

Brexpiprazole is cleared via CYP2D6 and CYP3A4, and desipramine is a weak to moderate CYP2D6 inhibitor that may raise brexpiprazole exposure. Exposure rises more if a CYP3A4 inhibitor is also given. Additive sedation and orthostatic effects can also occur.

What this interaction means in plain words

Brexpiprazole and Desipramine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Brexpiprazole belongs to Central Nervous System; Desipramine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Inhibition of CYP2D6-mediated brexpiprazole metabolism plus additive CNS depressant effects.

What to do

Monitor for brexpiprazole adverse effects such as akathisia, sedation and orthostasis, and reduce its dose if needed; follow labeling for dose reduction when combined with both CYP2D6 and CYP3A4 inhibitors.
