Summary

Imatinib is a CYP3A4 inhibitor and may increase brigatinib concentrations. Higher exposure raises the risk of brigatinib adverse effects such as pulmonary toxicity, hypertension, bradycardia and pancreatic enzyme elevation.

What this interaction means in plain words

Brigatinib and Imatinib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Brigatinib belongs to Oncology; Imatinib belongs to Oncology.

Evidence level: Moderate

Why it happens

Imatinib inhibits CYP3A4-mediated metabolism of brigatinib.

What to do

Prefer an alternative to imatinib where possible. If it is needed, reduce the brigatinib dose by about half and monitor for respiratory symptoms, heart rate and blood pressure; restore the earlier dose after imatinib is stopped.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
