Summary

Cobicistat is a strong CYP3A4 inhibitor and may considerably raise bromocriptine exposure. This can cause nausea, hypotension, syncope and other dopaminergic effects, and ergot-related toxicity is a concern with potent CYP3A4 inhibitors.

What this interaction means in plain words

Bromocriptine and Cobicistat can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Bromocriptine belongs to Central Nervous System; Cobicistat belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4-mediated bromocriptine metabolism by cobicistat.

What to do

Start with a low bromocriptine dose and titrate carefully, or choose an alternative dopamine agonist. Monitor blood pressure and for signs of ergot toxicity.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
