Bromocriptine is a dopamine agonist and thioridazine is a dopamine antagonist, so each opposes the other's effects. Thioridazine can reduce bromocriptine's benefit in Parkinson's disease, hyperprolactinemia or acromegaly, while bromocriptine may worsen psychosis.
What this interaction means in plain words
Bromocriptine and Thioridazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Bromocriptine belongs to Central Nervous System; Thioridazine belongs to Central Nervous System.
Why it happens
Pharmacodynamic antagonism at dopamine D2 receptors.
What to do
Avoid the combination when possible; use an antipsychotic with less D2 blockade such as quetiapine or clozapine in Parkinson's disease. Monitor prolactin or symptom control and psychiatric status if both are required.
