Summary
Bumetanide is a loop diuretic and adds to the diuretic and potassium-wasting effects of the hydrochlorothiazide component. This can cause hypokalemia, hyponatremia, hypomagnesemia, volume depletion and hypotension, especially in elderly patients or those on a low-salt diet.
What this interaction means in plain words
Bumetanide and Eprosartan-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Eprosartan-hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Additive natriuresis and urinary potassium loss from sequential blockade of sodium transport in the loop of Henle and the distal tubule, with added volume depletion alongside angiotensin receptor blockade.
What to do
Monitor serum potassium, sodium, magnesium and renal function after starting or changing doses, and watch for orthostatic hypotension. Replace electrolytes as needed and reduce diuretic doses if volume depletion develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
