Summary
Loop and thiazide diuretics together produce sequential nephron blockade, causing strong diuresis. This can lead to hypokalemia, hyponatremia, hypomagnesemia, volume depletion and renal impairment.
What this interaction means in plain words
Bumetanide and Hydrochlorothiazide-telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Hydrochlorothiazide-telmisartan belongs to Cardiovascular.
Why it happens
Additive natriuresis and potassium loss from blocking sodium reabsorption at two nephron sites, with added volume depletion alongside ARB therapy.
What to do
Monitor electrolytes, renal function and volume status within the first weeks and after dose changes; replace potassium and magnesium as needed and reduce diuretic doses if volume depleted.
