Summary
Bumetanide and hydrochlorothiazide both cause renal loss of potassium, sodium and magnesium, and together produce potent sequential diuresis. This may cause hypokalemia, hyponatremia, volume depletion and hypotension, and valsartan adds to the risk of renal impairment when volume depleted.
What this interaction means in plain words
Bumetanide and Hydrochlorothiazide-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Hydrochlorothiazide-valsartan belongs to Cardiovascular.
Why it happens
Additive natriuresis and kaliuresis from loop and thiazide diuretics acting at sequential nephron sites.
What to do
Monitor electrolytes, renal function, weight and blood pressure, and reduce diuretic doses if volume depletion develops. Replace potassium and magnesium as needed.
What to ask your pharmacist
