Summary

Lofexidine lowers blood pressure and heart rate, and bumetanide adds blood pressure reduction through potent diuresis. Together they can cause symptomatic hypotension and syncope. Bumetanide-induced hypokalemia or hypomagnesemia may also add to lofexidine's QT risk.

What this interaction means in plain words

Bumetanide and Lofexidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bumetanide belongs to Cardiovascular; Lofexidine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive hypotensive effects of central alpha-2 agonism and loop diuretic volume depletion, with electrolyte loss adding to QT risk.

What to do

Monitor blood pressure, pulse and electrolytes. Reduce the dose of either drug if symptomatic hypotension or bradycardia develops.
