Bumetanide + Ofloxacin

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Bumetanide can cause hypokalemia/hypomagnesemia, which may potentiate the QTc-prolonging effect of ofloxacin and increase the risk of torsades de pointes or other ventricular arrhythmias.

Mechanism of action

Pharmacodynamic: electrolyte depletion (K+, Mg2+) from bumetanide predisposes to QT prolongation; ofloxacin has QT-prolonging potential. No significant pharmacokinetic interaction known.

Clinical management

Correct and maintain electrolytes (target K > 4 mEq/L, Mg > 2 mg/dL) before/during coadministration; obtain a baseline ECG and repeat if additional risk factors (elderly, structural heart disease, bradycardia, high doses, renal impairment) or symptoms arise; avoid other QT-prolonging agents when possible; consider a non–QT-prolonging antibiotic if significant baseline QT risk or refractory hypokalemia.
