Summary
Loop diuretics are commonly combined with sacubitril-valsartan in heart failure. The diuretic promotes potassium loss while the angiotensin receptor blocker reduces aldosterone-mediated potassium excretion, so potassium effects may offset. Excess diuresis can also cause volume depletion, hypotension and reduced renal function.
What this interaction means in plain words
Bumetanide and Sacubitril-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Sacubitril-valsartan belongs to Cardiovascular.
Why it happens
Opposing effects on potassium handling plus additive hypotensive and volume-lowering pharmacodynamic effects.
What to do
Monitor serum potassium, creatinine, blood pressure and volume status after starting or changing doses. Reduce the diuretic dose if hypotension or volume depletion develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
