Summary
Starting an ACE inhibitor in a patient on a loop diuretic can produce first-dose hypotension and acute deterioration in renal function, especially with volume depletion. The risk is greatest early in therapy or after diuretic dose increases.
What this interaction means in plain words
Bumetanide and Trandolapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Trandolapril belongs to Cardiovascular.
Why it happens
Diuretic-induced volume and sodium depletion activates the renin-angiotensin system, so ACE inhibition causes exaggerated hypotension and reduced glomerular filtration.
What to do
Consider temporarily lowering or holding the diuretic before starting trandolapril and begin with a low dose. Check blood pressure, creatinine and potassium within the first one to two weeks.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
