Summary

Fosaprepitant is converted to aprepitant, which is a moderate CYP3A4 inhibitor. A single IV dose causes only a short-lived rise in exposure to CYP3A4 substrates such as buspirone, which may bring on more dizziness, drowsiness or nausea.

What this interaction means in plain words

Buspirone and Fosaprepitant have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Buspirone belongs to Central Nervous System; Fosaprepitant belongs to Gastrointestinal Tract.

Evidence level: Moderate

Why it happens

Aprepitant temporarily inhibits CYP3A4-mediated first-pass and hepatic metabolism of buspirone.

What to do

No routine dose change is needed for a single fosaprepitant dose. Watch for excess sedation or dizziness and consider reducing the buspirone dose if symptoms occur.
