Severity: Moderate

Calcium carbonate and Sodium polystyrene sulfonate: moderate interaction.

Summary

Sodium polystyrene sulfonate can bind calcium in the gut, and the antacid effect of calcium carbonate may be offset by cation exchange. Reduced binding of bicarbonate-producing cations has been associated with metabolic alkalosis, especially in renal impairment. Clinical impact is usually modest but can matter in dialysis patients.

What this interaction means in plain words

Calcium carbonate and Sodium polystyrene sulfonate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Calcium carbonate belongs to Gastrointestinal Tract; Sodium polystyrene sulfonate belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Non-absorbed cation-exchange resin binds calcium and other cations in the gut, and combined with non-absorbable antacid it can leave bicarbonate unneutralized and cause systemic alkalosis.

What to do

Separate administration by at least 3 hours (6 hours in gastroparesis) and monitor serum calcium, potassium and acid-base status in renal impairment.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
