Summary

Heparins, including low molecular weight heparins, can suppress aldosterone secretion and cause hyperkalemia. This effect is additive with ARBs, which also reduce aldosterone-mediated potassium excretion.

What this interaction means in plain words

Candesartan-hydrochlorothiazide and Dalteparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Candesartan-hydrochlorothiazide belongs to Cardiovascular; Dalteparin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin-induced inhibition of adrenal aldosterone synthesis adds to angiotensin receptor blockade, reducing potassium excretion.

What to do

Monitor serum potassium, particularly with prolonged use, renal impairment or diabetes. The thiazide partly offsets the effect but does not remove the need for monitoring.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
