Summary
Torsemide is a loop diuretic that adds to the diuresis and potassium and magnesium loss of hydrochlorothiazide, producing sequential nephron blockade. This can cause marked volume depletion, hypotension, electrolyte disturbance and renal impairment, particularly with an ARB on board.
What this interaction means in plain words
Candesartan-hydrochlorothiazide and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Candesartan-hydrochlorothiazide belongs to Cardiovascular; Torsemide belongs to Cardiovascular.
Why it happens
Additive natriuresis and electrolyte loss from loop and thiazide diuretics, with ARB reducing renal compensatory mechanisms.
What to do
Monitor electrolytes, creatinine, weight and blood pressure. Adjust diuretic doses if volume depletion or hypokalemia occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
