Summary

Volatile anesthetics such as desflurane cause vasodilation and myocardial depression, and patients taking ACE inhibitors and diuretics may experience pronounced hypotension during induction and maintenance of anesthesia. Volume depletion from the thiazide can make the hypotension worse.

What this interaction means in plain words

Captopril-hydrochlorothiazide and Desflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Captopril-hydrochlorothiazide belongs to Cardiovascular; Desflurane belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive vasodilation and blunting of compensatory renin-angiotensin responses during inhalational anesthesia, with volume depletion from the diuretic.

What to do

Inform the anesthesia team about the antihypertensive regimen and consider holding the ACE inhibitor/diuretic on the morning of surgery per local protocol. Monitor blood pressure continuously and treat hypotension with fluids or vasopressors.
