Summary
Hydrochlorothiazide-induced electrolyte disturbances such as hypokalemia and hypomagnesemia may coincide with gentamicin-related renal tubular toxicity. Gentamicin also carries nephrotoxic and ototoxic risk that is increased by volume depletion from diuretics and by ACE inhibitor effects on renal perfusion.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Gentamicin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Captopril-hydrochlorothiazide belongs to Cardiovascular; Gentamicin belongs to Anti Bacterial.
Why it happens
Additive electrolyte wasting plus diuretic- and ACE inhibitor-related reductions in renal perfusion increase aminoglycoside nephrotoxicity.
What to do
Monitor serum creatinine, potassium and magnesium during gentamicin therapy and check gentamicin levels. Correct electrolyte and volume deficits and consider holding the diuretic during acute illness.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
