Summary
NSAIDs such as lornoxicam can blunt the blood pressure lowering effect of ACE inhibitors and thiazides. Together with these drugs they raise the risk of acute kidney injury, particularly in volume-depleted or elderly patients, and can contribute to hyperkalemia.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Lornoxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Captopril-hydrochlorothiazide belongs to Cardiovascular; Lornoxicam belongs to Musculoskeletal.
Why it happens
Inhibition of renal prostaglandin synthesis causes sodium and fluid retention and reduces renal perfusion, opposing antihypertensive and diuretic effects and impairing glomerular filtration.
What to do
Monitor blood pressure, serum creatinine and potassium, especially at the start of therapy, and ensure adequate hydration. Use the lowest effective NSAID dose for the shortest time or choose an alternative analgesic.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
