Summary
Adding a loop diuretic to an ACE inhibitor and thiazide regimen produces strong diuresis and sequential nephron blockade. This can cause marked hypotension, particularly after the first doses, and may cause acute kidney injury in volume-depleted patients. Electrolyte disturbances are also more likely.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Captopril-hydrochlorothiazide belongs to Cardiovascular; Torsemide belongs to Cardiovascular.
Why it happens
Additive pharmacodynamic effects: diuretic-induced volume depletion combined with ACE inhibitor loss of angiotensin II-mediated efferent arteriolar tone.
What to do
Monitor blood pressure, renal function and electrolytes, especially at initiation or dose changes. Consider lowering the diuretic dose if the patient is volume depleted.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
