Summary
Celecoxib can reduce the antihypertensive effect of losartan and, together with the ARB, may impair renal function, especially in elderly, dehydrated or renally impaired patients. Hyperkalemia risk is also increased.
What this interaction means in plain words
Celecoxib and Losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Celecoxib belongs to Musculoskeletal; Losartan belongs to Cardiovascular.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins reduces glomerular perfusion while ARB blocks efferent arteriolar tone, causing reduced GFR, fluid retention and potassium retention.
What to do
Monitor blood pressure, serum creatinine and potassium after starting the combination and periodically afterwards. Maintain hydration and avoid the combination in patients with significant renal impairment or volume depletion.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
