Drug interactions
Celecoxib + Rivaroxaban
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Additive bleeding risk (including GI and extracranial) when rivaroxaban is used with NSAIDs such as celecoxib; COX-2 selectivity lowers but does not eliminate GI bleeding risk.
Mechanism of action
Pharmacodynamic: combined anticoagulation from rivaroxaban with NSAID-associated impairment of mucosal integrity and hemostasis; no clinically meaningful pharmacokinetic interaction expected.
Clinical management
Avoid concurrent use when possible. If an NSAID is necessary, prefer the lowest effective celecoxib dose for the shortest duration and add gastroprotection (e.g., PPI). Counsel and monitor closely for bleeding (bruising, melena, hematuria); consider baseline and periodic CBC and assessment of renal function. Avoid additional antithrombotics or other bleeding-risk drugs.
