Summary
Ceritinib inhibits CYP3A4 and may raise cyclosporine levels, while cyclosporine may increase ceritinib exposure through P-gp and CYP3A4 inhibition. Both effects can increase toxicity, including nephrotoxicity and QT prolongation.
What this interaction means in plain words
Ceritinib and Cyclosporin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ceritinib belongs to Oncology; Cyclosporin belongs to Miscellaneous.
Why it happens
Ceritinib inhibits CYP3A4, and cyclosporine inhibits P-glycoprotein and CYP3A4, so each can increase the other's exposure.
What to do
Monitor cyclosporine trough levels and renal function, and monitor ECG and electrolytes for ceritinib. Reduce the dose of either drug if toxicity develops.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
