Summary
Ceritinib is a strong CYP3A4 inhibitor and may raise salmeterol and fluticasone exposure. Higher salmeterol levels may cause tachycardia, tremor, hypokalemia and QT prolongation, and ceritinib itself prolongs QT.
What this interaction means in plain words
Ceritinib and Fluticasone-salmeterol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ceritinib belongs to Oncology; Fluticasone-salmeterol belongs to Respiratory.
Why it happens
CYP3A4 inhibition decreasing salmeterol clearance, plus additive QT prolongation.
What to do
Avoid the combination if possible, or monitor ECG, heart rate and potassium closely. Consider an alternative bronchodilator not dependent on CYP3A4 metabolism.
