Summary

Ceritinib prolongs the QT interval and inhibits CYP3A4, the main enzyme that clears pimavanserin. Pimavanserin levels may therefore rise, increasing its own QT effect on top of the additive effect of ceritinib.

What this interaction means in plain words

Ceritinib and Pimavanserin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Ceritinib belongs to Oncology; Pimavanserin belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive QT prolongation plus CYP3A4 inhibition by ceritinib, which reduces pimavanserin clearance.

What to do

Avoid the combination where possible. If it is unavoidable, reduce the pimavanserin dose, monitor ECG and electrolytes, and watch for arrhythmia symptoms.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
