Summary
Eliglustat is metabolized by CYP2D6 and CYP3A4, and CYP3A inhibitors raise its exposure, which may prolong PR, QTc and QRS intervals. The risk is greater in CYP2D6 poor or intermediate metabolizers.
What this interaction means in plain words
Chloramphenicol and Eliglustat can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Chloramphenicol belongs to Anti Bacterial; Eliglustat belongs to Miscellaneous.
Why it happens
Inhibition of CYP3A4-mediated metabolism of eliglustat, increasing its plasma concentrations.
What to do
Avoid combining in patients who are CYP2D6 poor or intermediate metabolizers, and use another antibiotic where possible. If used, monitor ECG and consider eliglustat dose reduction.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
