Summary

Fedratinib is metabolized by CYP3A4 and to a lesser extent by CYP2C19, and its levels rise with enzyme inhibition. Higher exposure may increase gastrointestinal toxicity, myelosuppression and the risk of encephalopathy including Wernicke's.

What this interaction means in plain words

Chloramphenicol and Fedratinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Chloramphenicol belongs to Anti Bacterial; Fedratinib belongs to Oncology.

Evidence level: Moderate

Why it happens

Chloramphenicol inhibits CYP3A4 and CYP2C19, reducing fedratinib metabolism.

What to do

Avoid if possible. If necessary, reduce the fedratinib dose per labeling for strong CYP3A4 inhibitors and monitor blood counts, thiamine status and neurologic symptoms.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
