Summary

Escitalopram and chlorpromazine each prolong the QT interval in a dose-related way. Together they raise the risk of torsades de pointes, particularly at higher doses or with other risk factors.

What this interaction means in plain words

Chlorpromazine and Escitalopram can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Chlorpromazine belongs to Central Nervous System; Escitalopram belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive QT prolongation by blockade of cardiac potassium channels; chlorpromazine may also modestly raise SSRI exposure via CYP2D6 inhibition.

What to do

Avoid if possible or keep escitalopram at the lowest effective dose (max 20 mg/day, lower in older adults). Monitor ECG and electrolytes and consider an alternative antidepressant.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
