For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Lonafarnib is a sensitive CYP3A4 substrate, and crizotinib is a moderate CYP3A4 inhibitor, so lonafarnib exposure may increase considerably. This raises the risk of toxicity such as gastrointestinal effects, hepatotoxicity and QT prolongation.

What this interaction means in plain words

Crizotinib and Lonafarnib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Crizotinib belongs to Oncology; Lonafarnib belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Crizotinib inhibits CYP3A4-mediated metabolism of lonafarnib.

What to do

Avoid the combination in line with lonafarnib labeling for CYP3A inhibitors. If it cannot be avoided, a specialist should guide dose reduction and monitoring.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
