Summary
Epcoritamab induces cytokine release that can temporarily suppress CYP450 enzymes. Cyclosporine exposure may therefore increase, raising the risk of nephrotoxicity, hypertension and neurotoxicity. The effect is most likely around cytokine release syndrome episodes.
What this interaction means in plain words
Cyclosporin and Epcoritamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Cyclosporin belongs to Miscellaneous; Epcoritamab belongs to Oncology.
Why it happens
Cytokine-mediated suppression of CYP3A4 activity decreases cyclosporine clearance.
What to do
Monitor cyclosporine trough levels and renal function when starting epcoritamab and during CRS. Adjust the cyclosporine dose according to levels and toxicity.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
