Summary
Heparins, including low molecular weight heparins, can suppress aldosterone secretion and cause hyperkalemia, which may be amplified by ACE inhibitors. The risk is greatest in renal impairment, diabetes or prolonged treatment. The thiazide component partly offsets potassium retention.
What this interaction means in plain words
Dalteparin and Enalapril-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dalteparin belongs to Cardiovascular; Enalapril-hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Heparin-induced inhibition of aldosterone synthesis adds to ACE inhibitor reduction of aldosterone.
What to do
Monitor serum potassium and renal function, especially with therapy longer than a few days or in patients with risk factors.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
