Summary

Heparins including low molecular weight heparins can suppress aldosterone secretion and cause hyperkalemia, which adds to the potassium-retaining effect of losartan. The risk is highest in renal impairment, diabetes and with prolonged use; the thiazide partly counteracts it.

What this interaction means in plain words

Dalteparin and Hydrochlorothiazide-losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dalteparin belongs to Cardiovascular; Hydrochlorothiazide-losartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin-induced inhibition of adrenal aldosterone synthesis combined with angiotensin receptor blockade increases potassium retention.

What to do

Monitor serum potassium and renal function, particularly if dalteparin is used for more than a few days. Adjust therapy if hyperkalemia develops.

What to ask your pharmacist
