Summary

Combining a low molecular weight heparin with a potent P2Y12 platelet inhibitor increases the likelihood of bleeding, including gastrointestinal and procedural bleeding. The pairing is often intentional in acute coronary syndromes, but the bleeding risk is still higher than with either drug alone.

What this interaction means in plain words

Dalteparin and Ticagrelor can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dalteparin belongs to Cardiovascular; Ticagrelor belongs to Cardiovascular.

Evidence level: High

Why it happens

Additive pharmacodynamic effect: dalteparin potentiates antithrombin-mediated inhibition of factor Xa while ticagrelor blocks ADP-induced platelet aggregation.

What to do

Use only when both agents are clearly indicated, and limit the duration of overlap. Monitor for bleeding signs and track hemoglobin and platelet counts.

What to ask your pharmacist

  • Should I take them at different times of day?
