Summary
Both dapsone and primaquine are oxidant drugs that can cause hemolytic anemia in G6PD deficiency and methemoglobinemia in other patients. Together the oxidative stress is additive, and the risk of hemolysis is higher.
What this interaction means in plain words
Dapsone and Primaquine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dapsone belongs to Anti Bacterial; Primaquine belongs to Anti Parasitic.
Why it happens
Additive oxidative stress on red blood cells from the hydroxylamine metabolites of dapsone and primaquine.
What to do
Test G6PD status before using either drug and avoid the combination in G6PD-deficient patients. If both are needed, monitor hemoglobin, methemoglobin and signs of hemolysis closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
