Summary
Apalutamide is a strong inducer of CYP3A4 and also induces CYP2C19, enzymes involved in dapsone metabolism. Dapsone concentrations may fall, which could reduce efficacy. Induction may also increase formation of hydroxylamine metabolites linked to methemoglobinemia and hemolysis.
What this interaction means in plain words
Apalutamide and Dapsone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Apalutamide belongs to Endocrine; Dapsone belongs to Anti Bacterial.
Why it happens
Apalutamide induces CYP3A4 (and CYP2C19), increasing dapsone clearance and possibly altering metabolite formation.
What to do
Monitor for loss of dapsone efficacy and for methemoglobinemia or hemolysis with blood counts. Consider an alternative agent if efficacy cannot be assured.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
